Formal Observation Process and forms:

1.  THE Evaluator contacts THE teacher to begin Process.

2.  THE Teacher complete pre-conference form.

3.  pre-Conference meeting CONDUCTED TO REVIEW PRE-CONFERENCE         
      FORM AND  Schedule Formal Observation.

4.  Observation (at least 30 min.)
       THE Evaluator completes Observation Form during observation.

5.  THE Teacher completes Post conference Self-Reflection Report.

6.  Post Conference Meeting (within 10 days of observation).                       
      (Observation Form and self-Reflection reports will be shared
       and discussed at this meeting.)
