Portfolio instructions:

1.  Create a folder on your computer or in Google Drive to save      
      your portfolio.

2.  Collect artifacts to provide evidence for each domain.
      (total = 12-24 artifacts)  LIST OF POSSIBLE ARTIFACTS

3.  download and complete the forms for each artifact.
      domain 1          domain 2          domain 3          domain 4

4.  INCLUDE your individual growth plan  in portfolio.

5.  submit portfolio (in digital or paper form) to THE
      administrator/EVALUATOR  by February 1st of the year you are 
      being evaluated.
